The following is a brief explanation of some of the services provided by the Utah School Employees Association for the exclusive benefit of members and their families.
INSURANCE PROGRAMS
Term
Life Insurance
USEA has arranged for a special $5,000 life insurance benefit plan for members of the
Association and their families. The plan pays $5,000 upon the death of the employee or
$10,000 if the death is related to an accident. Coverage for spouse and children is
$2,000. The low monthly cost for the benefit is $3.00 for the employee or $4.00 per month
for the employee, spouse and children.
Return of
Dues
The return of dues benefit will pay to a member’s beneficiary the amount of state
dues paid to USEA for membership for the fiscal year up to the month of the member’s
death. This benefit is provided at no additional cost to the member.
Mini
Med Program
USEA is
offering a medical insurance plan to help our members who do not have
access to school district sponsored group health insurance. The plan,
American Medical Access Plan, offers basic health coverage, doctor office
visits, x-rays/lab tests, prescriptions, and hospitalization benefits. The
plan is voluntary, so members who enroll in the plan are responsible for
the full premium amount. For your convenience, payment will be deducted
from your payroll through Educators Mutual Insurance Association (EMIA).
Supplement to
Medicare
Members of the Association who are retiring may choose to enroll in a supplement to
Medicare program, which is offered through Educators Mutual.
Medical Insurance Conversion Plan
Members who may no longer be covered by group medical insurance through their school
district may enroll in conversion programs offered by Educators Mutual to members of USEA.
Several different plan choices are available, depending on the needs of the member.
Dental Select
In an effort to improve benefits available to members, we have evaluated
several different dental insurance companies. Keeping your suggestions in
mind, we found the "Educational Dental Benefit Pool" administered
by Dental Select to be the best all around dental insurance company. Dental
Select offers two great plan options to choose from with low premiums,
excellent overall benefits, high allowable amounts, excellent claims
